The Health Equity Navigator Program in District 4 has been operating in Butts, Carroll, Coweta, Fayette, Heard, Henry, Lamar, Meriwether, Pike, Spalding, Troup, and Upson counties for the last two and a half years. During this period, the Navigators have conducted surveys in these counties to gain a deeper insight into the communities’ needs. They have collaborated with Family Connections Agencies and Health Departments in these areas to enhance community service.
The Navigators successfully pinpointed the resources accessible in every county. They collaborated in collecting data such as organization and agency names, services offered, addresses, contact details, and website links when applicable.
The Health Equity Navigator Program has compiled a comprehensive resource guide containing information on all counties with available resources. This document will feature national and state resources, as well as details on organizations and agencies related to health and wellness, mental health and substance abuse, senior services, social services, special assistance, community services, child services and youth development, veteran resources, employment, financial and legal resources, housing services, food, clothing, utilities, and transportation. Access all this valuable information with ease.
